He was predeceased by: his parents, Benedict Kozlowski and Julia Kozlowski; and his brother Richard Kozlowski. He is survived by: his wife Carole; his sons, Ben Kozlowski (Kim) and Dennis Kozlowski (Kimberly); and his granddaughters, Nicole and Jennifer.

At the family's request, all guests will be asked to wear a mask.
