She was predeceased by: her husband Leonard Forman; her son Michael; and her brothers, Joseph and James Eastwood. She is survived by: her children, Jeffrey Forman, Brenda Forman and Lynn Plizga (Greg); and her grandchildren, Ann Forman, Thomas Forman and Mikayla Martin.
