He was predeceased by: his wife Margaret A. Cook; his parents, Willie Cook Sr. and Mattie B. Cook; his son Thomas Willie Ruffner; and his siblings, Rev. David Cook Sr., Jackie Cook, Alonzo A Cook, Jimmie Cook, Stevie Cook, Geneva Johnson and Annie Willie Howell.
He is survived by: his children, Cathy Cook, Teresa Cook, Lisa M. Cook, Jennifer Duncan (Jediah D. Duncan), Chanel Cook, Willie Cook III (Yanylieen Rodriguez) and Jerry Cook; and his sister Rose Crawl. He is also survived by 11 grandchildren and 6 great grandchildren, in addition to a host of nieces and nephews, relatives and friends.
