He was predeceased by: his parents, Thomas Hazelton, Sr. and Emma McCabe Hazelton Thomas; and his siblings, Leonard Hazelton and Grace Leese. He is survived by: his wife Debra Moran; his children, Patricia McGuire, Chrystal Hazelton, Amanda Beader, Holly Hazelton, Donald Hazelton, Jr. (Michele), Hayley Hazelton, Antonio Hazelton, William Hazelton and Tiana Hazelton; and his brother Thomas Hazelton, Jr.. He is also survived by 16 grandchildren; 5 great-grandchildren.
Visitation was held on Monday, September 15th 2025 from 5:00 PM to 8:00 PM at the Carmine J. & Louis C. Parise Funeral Home & Cremation Services, Inc. (89 Farview St, Carbondale, PA 18407). A funeral service was held on Tuesday, September 16th 2025 from 10:00 AM to 11:00 AM at the same location. A burial was held on Tuesday, September 16th 2025 from 11:30 AM to 12:00 PM at the Our Mother of Sorrows Cemetery (400 PA-106, Greenfield Township, PA 18407).
