He was predeceased by: his parents, Harry W. Sample and Elaine Sample (Hooper); his sister Cheryl Phython (Barry); and his in-laws, JC and Becky Gardner.
He is survived by: his wife Jean Gardner; his sons, Jason (Christy) and Jeremy (Aimee); his grandchildren, Brayden, Riley, Raegan and Jayden; his brother Jim (Mary); his nieces and nephews, Carrie Wagner (John), Amy Phythyon, Brooke Sample (Richard Quinn), Shawn Phythyon, Brent and Drew Godfrey (Pam); and his sister-in-law Sharon Godfrey.
