He is survived by: his children, Jason Futoma (Amy) of Shickshinny and Melissa Hook (Larry Jr.) of Mountain Top; his brother Thomas Futoma (Debbie Futoma) of Shickshinny; and his grandchildren, Madison Futoma of Mountain Top, Hannah of Mountain Top and Noah Hook of Mountain Top.
