He was predeceased by: his parents, Theodore Van Zelst, Jr. and Kathleen M. Van Zelst. He is survived by: his sisters, Kathleen M. Smith and Dorothy M. Gemmell (John); his niece Michelle; and his nephew Sean. He is also survived by 1 great-niece and 2 great-nephews.
