He was predeceased by: his parents, Stanley T. Sr. Werkheiser and Mary Werkheiser (Tiger); and his sister Judy. He is survived by: his wife Patricia Werkheiser (Metz); his daughters, Shannon Johnson (Chris) and Carissa Werkheiser; his grandchildren, Ava, Adelyn, Ace and Ariella; his siblings, Stanley, Barbara, Johnny, Barry, Lester and Jenny; his siblings-in-law, Guy and Brenda; and his niece Julie.

In lieu of flowers, the family kindly requests contributions to help offset costs associated with the athletic endeavors of Donald's grandchildren.
