He was predeceased by: his parents, Claude C. "Dick" Clark and Sarah Clark (Lundy); and his nephew Michael Keith Clark. He is survived by: his girlfriend Susan Salzman Hoover of Herrick Township; his brother Keith William; his sister-in-law Michelle R. Clark of Herrick Township; his nieces, Michelle Andersen (Evan) and Sylvia Danielle Clark; and his great-nieces and great-nephews, Taelyn, Sarah, Seth, Elijah, Aiden and Liam. He is also survived by numerous cousins and many dear friends.
The family suggests that memorial contributions be made in Dale's memory to the Herrickville Volunteer Fire Company, HOPS Road, Wyalusing, PA 18853, or to the H.O.P.S. Ambulance Association, 6185 Herrickville Road, Wyalusing, PA 18853.
