She was predeceased by: her parents, Frank Kokoszka and Mildred Woronicak Kokoszka; her husband Anthony Zalewski; her sons, Kevin Zalewski Sr. and Dennis Michkelkiewicz; and her brothers, Frank and John Kokoszka.
She is survived by: her children, David Zalewski Sr. and Dawn Phillips; her sister Ann Lane; and her grandchildren, Kevin Jr., David Jr., Anthony, Candice, Jason, Taylor and Ian.
