He was predeceased by: his parents, John W. Koslick and Ann Marie Koslick (Berardinis); his son Richard W Koslick Jr.; and his nephew Gerry Provance Jr.. He is survived by: his daughter Jennifer Koslick of Pittsburgh; his sisters, Linda Provance (Gerry) of Metarie, LA and Karen Koslick of Davenport, IA; his nephew James Rizer (Jill); his nieces, Deena "Belle" Couget (Ryan) and Jessica Williams; his nephew Michael Valerius; Dax.
