He was predeceased by: his parents, Agnes Kelly and John J. Henry. He is survived by: his wife Carol Henry; his daughter Denise Tecce (Joseph); his stepson Mark Allen (Mai); his stepgrandson Tyler Conrad; and his brother James Henry. He is also survived by numerous nieces and nephews.
In lieu of flowers, the family kindly requests that memorial contributions be made to one of Jack's cherished causes: Tunnel to Towers Foundation, St. Jude Children's Research Hospital, or Providence Animal Rescue.
