It is with great sadness that we announce the death of Douglas A. Hazen of Wampum, Pennsylvania, born in Ellwood City, Pennsylvania, who passed away on November 28, 2023, at the age of 58, leaving to mourn family and friends. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.

He was predeceased by : his parent Ellwood L. "Woody" Hazen. He is survived by : his mother Beverly A. Ferrante Hazen of Ellwood City; his daughter Samantha Ann Lavoie (James) of New Hampshire; and his grandsons, Gavin and Nolan Lavoie. He is also survived by several aunts, uncles, cousins, and good friends.

In lieu of flowers. Memorial Donations in Douglas A. Hazen's name may be made to the Divine Grace Parish Social Services.
