He was predeceased by: his parents, Elmer C. "Corky" Slack and Valjean F. Rudolph Slack; and his sister Diane. He is survived by: his children, David Slack (John Wolf) of Erie, PA and Emily Heeter (Mark) of Sheffield, PA; his granddaughters, Addisen and Peyton; and his sisters, Debbie and Denise. He is also survived by many nieces and nephews.
