He was predeceased by : his mother Mary "Molly" Hendrick Horniak. He is survived by : his father Stephen Horniak; his sons, Aydan and Chase Horniak; his siblings, Stephen Horniak (Cathy) and Jessica Sklar; his nieces, Cassidy and Emily Horniak; and his nephew Alex Sklar (Megan Harkins). He is also survived by many extended family and friends.

“We met on a golf course a few years back, and he had game. When we played I discovered he had had three passions: The love for his two sons, guitars and golf. We traded stories big and small and always, those three passions game out in every conversation. I moved out of the area and we chatted a few times in the subsequent years and then the phone calls faded. I was saddened to hear of his passing. My kindest regards to his family and friends at this difficult time. Manny Amatrudo”
