We are sad to announce that, on January 21, 2024, at the age of 86, Hazel Elaine Chadwick of Washington, Pennsylvania passed away. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.
She was predeceased by: her parents, Harry M. Sampson and Thelma A. Sampson; her daughter Linda Crowe; and her grandchildren, Shawn Chadwick, Damian Chadwick and Ryan Phillips.
She is survived by: her grandchildren, Nathan Chadwick, Justin Chadwick (Kandyce), Lisa Yoders (Chris), Daysha Love (Kyle) and Nicole Crowe; and her siblings, Marlene Webb (James), Zelda Jefferies (Paul), Shirley Jankowski, Ola Jackson and Howard. She is also survived by twelve great-grandchildren; six great-great grandchildren; and a number of nieces and nephews.
