He was predeceased by: his parents, John Forman and Mary Forman (Godshall); his wife Anna G. Smoker; his grandson Jesse Forman; and his son-in-law Joseph Garreau.
He is survived by: his children, Stephen Forman (Rebecca) of Laurel, DE, Carl Forman (Joni) of Turbotville, Rosanna Hess (Nelson) of Toccoa, GA, Regena Garreau of New Columbia, Rebecca Cruise (Kelly) of Wooster, OH and Rhonda Formanek of Leipzig, Germany. He is also survived by 13 grandchildren; 24 great-grandchildren; and 4 great-great-grandchildren.
