He is survived by: his mother Mary Dvorak (Hladish); his siblings, Joseph (Anna Jo) of Prospect Park, John (Diane) of Still Creek, Mary Garzio (Mark) of Lake Hauto, Jerome (Janis) of McAdoo, Jene (Kerissa) of Palmyra, Jude (Sherry) of New York and Cathy Steffek (Cory) of Colorado; his girlfriend Andrea McAloose; his girlfriend's son Matthew McAloose; his nephews, Joseph and Michael Dvorak; and his great-nieces and great-nephews, Stella, Ryker, Knox, Zoey and Azalea. He is also survived by many aunts and cousins.
