He was predeceased by: his father Albert Polaski; and his brother Albart "A. J." Polaski. He is survived by: his mother Lea Polaski (Rios); his son Gavin Hoffman; his brother Curtis James Polaski; and his niece Adela Polaski. He is also survived by many loving aunts, uncles, and friends.
A reception will be held on Saturday, November 22nd 2025 from 12:00 PM to 2:00 PM at the Sperling Funeral and Cremation Care, Inc. (700 Blazier Dr, Wexford, PA 15090).
