He was predeceased by: his parents, John Costa and Josephine Costa (Sorrentine); his siblings, Marjorie Delmonico and Richard Costa; and his granddaughter Christina E. Caldas.
He is survived by: his children, Leonard F. Costa, Jr. of Cranston, Deborah A. Caldas, Zimmerman of Attleboro, Thomas J. Costa of East Providence, Steven J. Costa of North Providence, Michael A. Costa of East Providence and Diane M. Hollenrake of Riverside; and his brother Norman Costa of East Providence. He is also survived by many nieces and nephews.
