He was predeceased by: his parents, Sidney Cook and Mary Cook (Conn); and his brother Sidney Davis Cook. He is survived by: his wife Roberta Collins Cook of Pawtucket, RI; his children, Robyn Bellamy (Nick) of Pawling, NY, William Cook (Danielle) of Plainville, MA and Jennifer Alarie (Philip) of Warwick, RI; his grandchildren, Sam, Will Bellamy, Max, Hannah Alarie, Ben and Libby Cook; and his nieces and nephews, Wayne Collins (Melissa), Pam Hall (Peter), Tim Cook (Holly) and Catherine Cook (Anne).

In lieu of flowers, the family requests donations be made to the Church of the Good Shepherd, 490 Broadway, Pawtucket, RI 02860.
