He was predeceased by: his parents, Frank and Adele (Zotti); and his brother Frank Anzivino Jr.. He is survived by: his wife Paula Anzivino (Altruda); and his children, Kerri Castelli (Michael), Jacqueline Gaipo (David), John Anzivino Jr. (Malorie) and Christopher (Lori). He is also survived by 13 grandchildren.
