He is survived by: his spouse Shannon Rondeau; his sons, McLean Polak and Ethan; and his siblings, Barry Polak (Karen), Cynthia Dursin (Richard), Sueann Jones (Dennis), Beth Ellen Borzain (John), Teddie James Polak, Carrie Shifflet and Doreen Hogan (Rick Rondeau). He is also survived by many nieces and nephews.
