He was predeceased by: his parents, Albert "Libby" LaFazia and Angela LaFazia (Ferri); Evelyn Gould (Barszcz). He is survived by: his wife Mary Ann LaFazia (Murphy); his daughter Karen Simpson of West Warwick; his stepdaughter Deborah Rossi of North Providence; and his grandsons, David and Steven Simpson.
