He is survived by: his sons, Andre Applegate of Providence, Nathan Applegate of Providence and Tyler Applegate of Providence; his grandchildren, Jalen, Grayson and Sophia; and his brothers, Joseph Fortes, Ernest Rezendes, Anthony Rezendez, Richard Rezendez and Ronald Rezendez. He is also survived by his children, grandchildren, brothers, extended family, and friends.
A funeral service will be held on Sunday, October 18th 2026 from 2:30 PM to 4:00 PM and at 4:00 PM.
