He was predeceased by: his parents, Frederick A. Hazard and Allegra Perry Hazard; his children, Joye B. Hazard of Providence and Brett A. Hazard; and his siblings, Frederick Hazard Jr., Deborah Pierce and Denise Perry.
He is survived by: his wife Jeannette P. Hazard; his children, Dawn A. Perry, Andrea B. Armelin of Providence, Kaesha M. Hazard of Pensacola and William A. Hazard Jr. of Providence; his siblings, Steven, Allegra, Lavonne and Edna Hazard; his grandchildren, Shlonda Davis, Nichole Ferreira, Denashia Cochran, Brett Ferreira, Jordan Hazard-Bliss, David Perry, Jonathan Hazard-Nettles III and Sire Hazard; and his great grandchildren, Jermaine Brett Jones, Amani Jones and Oshin Perry.
