He was predeceased by: his parents, John and Victoria Lena Costa Sr. (Pagano); his sisters, Ann Marie Blouin and Rita C. Galinelli; Josette Squatrito Costa.

He is survived by: his sons, John Costa, III (Jana) of Johnston, RI and Kevin Costa (Jessica) of Bristol, RI; his grandchildren, Allie Costa, Cote Costa (Kaylee), Sophie Ross (Nolan), Andersyn Costa (Jarrett), Jon Logan Costa and Christian Costa; and his great grandchild Rhode Costa. He is also survived by several godchildren, nieces and nephews.
