She was predeceased by: her spouse Avery O. Cook; her parents, Charles Welles and Pauline Welles (Henry); and her sister Gloria Newsome. She is survived by: her sons, John Cook (Susan) of Franklin, Jeffery Cook (Fran) of Jamestown, Dean Cook (Annette) of Scituate and James Cook (Dee) of Warwick. She is also survived by eleven grandchildren; two great grandchildren.
