He was predeceased by: his parents, Anthony DeFazzio and Viola DeFazzio (Stabile); and his sister Mary Roy. He is survived by: his wife Sally Jean DeFazzio (Peckham); his stepchildren, Sean Manwaring (Michelle) and Elizabeth Clark; and his sister Lorraine DeFazzio. He is also survived by grandchildren.
A reception will be held on Tuesday, September 30th 2025 from 2:00 PM to 2:30 PM at the Carpenter-Jenks Funeral Home (659 E Greenwich Ave, West Warwick, RI 02893). Visitation will be held on Tuesday, September 30th 2025 from 2:30 PM to 4:00 PM at the same location. A prayer will be held on Tuesday, September 30th 2025 at 4:00 PM at the same location.
In lieu of flowers, donations in Peter's memory may be made to St. Jude Children's Research Hospital or the Rhode Island SPCA, honoring his love for children and animals.
