He was predeceased by: his mother Mary Cook. He is survived by: his father James N. Cook, Sr. of Richmond, RI; his wife Jacqueline Cook (Stanley) of Sisk; his son Dustin J. Cook (Brooke) of Westerly; his sisters, Tanya Paige of West Warwick and Tara Cook of Coventry; his grandson Adrian J. Cook; and his niece Erica Kilbourne of Coventry.
