He was predeceased by: his parents, Andrew Puhak and Helen Puhak; and his sister Theresa Cassarella of Pennsylvania. He is survived by: his sister Marie Tolerico. He is also survived by many nieces, nephews and great nieces & nephews, and great great niece.
