He is survived by: his mother Mary D'Antonio Self; his aunts and uncles, Robert D'Antonio, Thomas Levin, Susan Gerber (Gary), Carol Vitton, Norma Kinnunen (Richard), Elizabeth Tessier (Mark), Barbara Roe, Linda DeRoo, Jeanette Kindred, Mary Esterholm and Debbie Semborski; and his godchildren, Jake and Baby Gary Thomas Gerber.
