He was predeceased by: his grandparents, Linda Gerard, Nancy Montley and Ernest Montley, Sr.. He is survived by: his parents, Anthony James Urban, Sr. and Zina Urban; his daughters, Ashlyn Marie Ford and Aubree Whitacre; his siblings, Anthony James Urban, Jr., Donald Sebastian Urban and Samantha Urban; his niece Ariel Urban; and his grandparent Jesse Urban. He is also survived by many other aunts, uncles and cousins.
