He was predeceased by: his parents, Michael Rutherford Cook and Sherry Bozeman Wells. He is survived by: his wife Michelle Hamilton Cook of Conway; his sons, Raleigh Cook of Conway and Ramsey Cook of Conway; his brother Andy Cook of Simpsonville, SC; his mother-in-law Linda Hamilton of Conway; and his brothers-in-law, Jody Hamilton (Suzy) of Conway and Matt Hamilton (Donna) of Aynor. He is also survived by nephews, a niece, and cousins.
