He was predeceased by: his parents, Oscar Lewis and Leona Inman Lewis; and his sisters, Janet Leona Burleson (Lewis) and Annie Neal Roberts. He is survived by: his children, Dustin Lewis and Kayla Lewis Cook (Blake Cook); his grandsons, Hunter Cook and Kye Cook; and his brothers, William "Carson" Lewis and Barry Lewis.
