He is survived by : his son Herman Lea Cook (Annette Huggins Cook); his parents, Herman Cook and Annette Cook; his children, Brandon Cook, Brittany Nicole Cook and Nicole Wallace; and his siblings, Debbie Grubbs (Robert), Janice Utz (Kenneth), J. P. Cook and Russell (Darlene). He is also survived by six grandchildren; two great-grandchildren.

The family has asked that in lieu of flowers that memorials may be given to Ghents Branch Baptist Church, Nehemiah Team, 1974 Ghents Branch Rd., Denmark, SC 29042.
