He was predeceased by: his parents, DeWitt Smith and Sara Cato. He is survived by: his significant other Linda; his daughters, Lindsay Mulderick (Jeremy) and Melissa Windham (Shane); his grandchildren, Ava Jane, Amelia, Stone Windham, James and Riley Mulderick; his sister Vicki Keller (Tim); and his brother-in-law Mike Allen (Kay). He is also survived by niece and nephews.
