He is survived by: his wife Mary Frances Cook; his daughters, Emily Carruth (Bryan), Leigh Bradberry (Steven) and Kelley Cook; his siblings, Cathy Bryant, Dewey Cook (Carol) and Alvin Cook (Betty); and his grandchildren, Chase Reid Carruth and Alekai Bradberry.
