Polk (Raven) of Cummings; his brothers, Lavern Polk (Carolyn) of Islandton and Timmy Polk (Barbara) of Islandton; his grandchildren, Landon Kipp Polk, Whitney James Polk and Colt Timothy Polk; his nephew Jared Polk (Carey); his great-nieces and great-nephews, Addison and Anderson; and his cousin Russell Gibson.
