He was predeceased by: his parents, Otis Lewis and Gertrude Lewis. He is survived by: his wife Andrea; his brothers, Mike (Sandra) and Keith (Cathy); and his children, David (Crissy), William, Phillip, Heather and Lisa. He is also survived by eight grandchildren, four great-grandchildren and numerous nieces and nephews.
