She was predeceased by: her husband Billy Ray Phillips; her parents, Edward Preston and Doris Winkle Mazyck; her brothers, William E. Mazyck and James R. Mazyck; and her nephew Jamie Mazyck.
She is survived by: her daughter Janice Phillips Todd; her grandchildren, Billy Ray Todd (Noel) and Robin Woodward (Ryan); her great grandchildren, Devon Hogue, Ryleigh Woodward and Madisyn Woodward; and her nieces and nephews, Eddie (Kimberly), Glen Mazyck, Dolores and Crystal Mazyck.
In lieu of flowers the family has asked that donations be made out to the ASPCA.
