He was predeceased by : his brother James "Buddy" Cook; his parents, James "Jimmy" Cook and Lois Smith Cook; his brother-in-law Charles Shirley; his sister Joyce Cook Shirley; and his pets, Fred and Skeeter.

He is survived by : his wife Debra Roxanna Wilson Cook; his sisters, Kathy Cook Krzemien of Rock Hill, SC and Carol Cook Burns (Bobby R. Burns) of York, SC; and his daughters, Angela Sisk Starnes (William Edward Starnes) and Kandi Holder Alvarez. He is also survived by many grandchildren, nieces, nephews, cousins, extended family, and friends.
