He is survived by: his wife Lisa Michelle Nettles Cook; his son Axl Cook (Kaylee); his stepchildren, Daniel Dobson and Megan Nix (Phillip); and his sisters, Melinda Cook (Mark Nettles) and Brenda Belger. He is also survived by nieces, nephews.
