He was predeceased by: his mother Marsha Chase (Foglesong). He is survived by: his father Lyman Chase; his wife Lana Chase; his children, Lukas (Autumn), Logan and Olivia; his sisters, Lori Day (Randy Lee), Teresa Summerfield and Susan Nagel (Roger); and his grandson Charles David Chase.
In Lieu of flowers the family requests memorials be made to the Eye On Grace Foundation that helps families alleviate the financial burden of treatments for Ocular/Uveal Melanoma.
