He was predeceased by: his parents, Frank Bultsma and Bettie Bultsma (Schelhaas). He is survived by: his wife Jana Birdsall; his siblings, Mari Kukral (Ron) and Daniel Bultsma (Debbie); his foster daughter Michelle Cazer (Rory); and his granddaughters, Kyme Blumer (Shaun) and Tamar Cazer. He is also survived by nieces and nephews.
