He was predeceased by: his parents, Ralph W. Lively and Linda Barker Lively. He is survived by: his wife Debi; his children, Lindsey Eberhardt (Dustan) and Anthony Lively; and his siblings, B. J. Lively (Julie) and Lisa Barnard. He is also survived by two nieces and a nephew; along with many cherished friends.
He has asked in lieu of flowers that a donation be made to St. Judes Children's Hospital in his honor.
