He was predeceased by: his parents, William Leslie and Susie Alcorn Mathis; his daughter-in-law Cindy Williams Mathis; and his siblings, Louise Walker and Dewey Mathis.
He is survived by: his sons, Johnny Mathis of Baxter and Jeff (Donna Mathis) of Baxter; his grandchildren, Chris Mathis (Tiffany) of Cookeville, Jessica Morgan (Josh) of Baxter and Zachary Brazle (Ashley) of Georgia; his great grandchildren, Kristian Mathis, Haylee Mathis, Libby and Finley Brazle; his sister-in-law Sue Mathis; and his niece Melissa Duke (Radford).
The family suggests memorials be made to Monroe Carell, Jr. Children's Hospital, 2200 Children's Way, Nashville, TN 37232.
