He was predeceased by: his parents, Clifford Church and Leeora Street Church. He is survived by: his wife Joann "Jody" Church; his children, Traci Harris (Brett) and Kelly Halley (Steve); his stepchildren, Annette Estes (Janet), Jack Estes (Candi) and John Estes (April); and his grandchildren, Nan Estes, Alex Estes, Xander Harris, Ryder Estes, Lincoln Estes, Kelshan Hensley, Kaileigh Hensley, Jaden Church-Heart, Addy James and Charlie James.
In lieu of flowers the family suggests that donations be made to St. Judes Children's Hospital.
