He was predeceased by: his wife Sue Ann Cook; his parents, Eugene Cook and Lillian Mae Dallas Farmer; and his siblings, Addie Mae, Faye, Ralph, Gene, Tommy and Harold.
He is survived by: his sons, Tony Cook and Tracy Cook; and his grandchildren, Shelby Webb (Brittan) and Ashley Cook.
