He is survived by: his wife Terri Lynn Blazak; his children, Kevin and Danielle; his brother Keith (Carol); his father-in-law Richard (MaryAnn Blazak); and his sister-in-law Kathy Chastain (Lynn). He is also survived by many nieces and nephews.
